Most destinations will require at least 6 months left on your passport please check you have this before booking. If you need to order another passport then you can look at Passport office website for the latest prices and time of delivery. http://www.ips.gov.uk/cps/rde/xchg/ips_live/hs.xsl/index.htm - Does the name on the passport need to match the name on the booking?
Yes the name you book online will need to match please make sure it does match as suppliers will charge for a name change. Even if you have just got married you still may need to enter your maiden name as you book to match your passport.
